How to Access Information
All residents, clients, patients, and participants at CapitalCare have a health record. These records contain health information such as reports from care providers, test results or information about medication. Health records can be paper, electronic or a combination of both.
How to Request Health Records
- Fill out the Request to Access Health Information Form or the Request to Access Information (FOIP) Form.
- Submit the completed form in one of the following three ways:
- mail to CapitalCare 21st Floor, 10004 – 104 Avenue, Edmonton, AB, T5J 0K1;
- fax to 780.496.7148;
- email to the Chief Information and Privacy Officer at info@capitalcare.net
All submissions require a clear copy of valid identification (ID). Please provide one of the following:
- One (1) piece of photo ID, e.g. Driver’s License, Passport, or ID Cards; or
- Two (2) pieces of non-photo ID, e.g. Health Card, Birth Certificate, Marriage Certificate.
All copies of ID will be destroyed in a confidential and secure manner once the request is processed.
- An administrative fee of $25.00 may be charged for handling an information request, including requests for personal health information. The initial administrative fee of $25.00 must accompany the request for access. Subsequent fees may be charged as calculated using the fee schedule of the FOIP Act.
- Requests for health information are usually processed within 30 calendar days from the date CapitalCare receives your request. CapitalCare will contact you if we need more information after receiving your request.
Requesting on Behalf of Another Person
When requesting information on behalf of another person, ensure you provide proof of your authority to act on behalf of another individual who is subject of the personal information (e.g. Written authorization, Authorizing a Representative form, Guardianship, Power of Attorney, etc.).
