How to Access Information

All residents, clients, patients, and participants at CapitalCare have a health record. These records contain health information such as reports from care providers, test results or information about medication. Health records can be paper, electronic or a combination of both.

Requests for health information are usually processed within 30 business days from the date CapitalCare receives your request. CapitalCare will contact you if we need more information after receiving your request

How to Request Health Information

  1. Fill out the Request to Access Health Information form
  2. Attach a copy of the required identification
    1. One piece of photo ID (e.g. Driver’s License, Passport, or ID Cards); or
    2. Two pieces of non-photo ID (e.g. Health Card, Birth Certificate, Marriage Certificate)

Note: All copies of ID will be destroyed in a confidential and secure manner once the request is processed.

  1. If requesting another individual’s health information:
    1. Complete the Consent to the Disclosure of Individually Identifying Health information form
    2. Include applicable supporting documentation (e.g. written authorization, Authorizing a Representative form, Guardianship, Power of Attorney)

If supporting documentation is unavailable or appropriate authority cannot be identified on the form, clearly indicate the purpose for the request, and the processing team will determine if the disclosure is permitted under the Health Information Act (HIA).

Fees

All requests will be charged a minimum fee of $25.
